Record each hazard, score it before and after your controls, and keep reviews on time.
For managers and carers
Write a risk assessment (managers and team leaders)
- Open the client's profile and go to the Risk Assessments tab.
- Press New Assessment.
- If you have templates, pick one under Start from a template. Otherwise fill in the Title and Category.
- Choose the Overall Risk Level: Low, Medium, High or Critical.
- Set the Next Review Date. You must enter one.
- Under Hazard Identification, press Add Hazard for each hazard. Fill in:
- What is the hazard? and Who might be harmed?
- Inherent Risk (before controls): likelihood and severity, each from 1 to 5.
- Control Measures: what staff do to reduce the risk.
- Residual Risk (after controls): the scores once those measures are in place.
- Responsible Person.
- Press Create Assessment.
Review an assessment
- Open the assessment from the Risk Assessments tab.
- If anything has changed, press Edit and update it.
- Press Mark Reviewed. The next review date moves on by the same gap as before.
Good to know
- Each hazard's score is likelihood times severity, out of 25.
- The overall risk level is chosen by you. It is not worked out from the hazard scores, so check it matches.
- The tab shows a banner when assessments are overdue for review.
- Carers can read risk assessments. Tell your manager if a risk has changed.
